Measure and Record Urinary Drainage Bag
Measuring and recording urinary drainage is an essential task in healthcare that provides valuable insight into a resident’s overall health and fluid balance. Accurate output monitoring helps detect early signs of dehydration, kidney dysfunction, urinary tract infections, and other serious medical conditions. For residents with catheters or other medical conditions affecting urination, this information is especially critical for guiding treatment decisions and ensuring timely interventions. Performing this task correctly also supports infection control and reinforces documentation accuracy—key components of safe, effective, and accountable care in any healthcare setting.
You will need the following when Measuring and Recording Contents of a Urinary Drainage Bag:
- Barrier
- Gloves
- Triangular Graduate Container
- Documentation Sheet
1. Preparation & Hand Hygiene
- Read and follow the Care plan, the Whole Care Plan, and nothing but the Care plan.
- Knock and announce yourself before entering.
- Greet the Resident by name and introduce yourself.
- Explain the task to the Resident and ensure their comfort and consent.
- Close the Privacy Curtain
- Wash your hands thoroughly before beginning.
2. Measure Urinary Drainage Bag
- Place Barrier on the floor, absorbent side up directly underneath the Urinary Drainage Bag.
- Put on a pair of Gloves.
- Retrieve Graduate Container from Bedside Drawer.
- Put gloves on BEFORE handling Graduate Container.
- Place Graduate Container on the Barrier, directly underneath the Urinary Drainage Bag.
- Remove T-Valve Port from its holder.
- Using the T-Valve Port, empty contents of Urinary Drainage Bag into Triangular Graduate.
- Do Not let T-Valve Port touch anything.
- Once the Urinary Drainage Bag is empty, close the T-Valve Port, and return the Port to it’s holder.
- Verify Resident’s Catheter Tubing is coiled on the bed, not coiled near the floor or underneath the Resident.
- Wrap Barrier around the Graduate Container.
- Transport Barrier/Graduate Container to the lid of the flat surface of the Bedside Commode/Toilet.
- Do Not set Graduate Container directly onto the Toilet/Bedside Commode. Must be on Barrier.
- Turn Graduate Container, so that the Measurement Numbers face you.
- Kneel to eye level with Graduate Containers Measurement numbers.
- Observe Fluid Level Measurement and make a Mental Note for recording.
3. Record Urinary Drainage Bag Measurement
- Lift the Lid of the Toilet/Bedside Commode.
- Empty Contents of Triangular Graduate into Toilet/Bedside Commode.
- Rinse Graduate Container in Sink, dispose of Rinsewater in Toilet/Bedside Commode.
- Clean Graduate Container per Facility Guidelines.
- Set Graduate Container in the sink.
- Use a paper towel to pick up the Graduate Container and another to dry the Container.
- Use a paper towel to open the Bedside Drawer, place Graduate Container in Bedside drawer and close.
- Dispose of paper towels.
- Dispose of your Gloves.
- Verify Resident’s Catheter Tubing is curled on the bed.
- NOT touching the Floor.
- NOT coiled under the Resident.
- Urinary Drainage Bag hanging from the bed.
- Ensure Resident is comfortable.
- Ask about Resident needs and preferences.
- Provide Resident with Call light.
- Open the Privacy curtain.
- Ensure bed is in lowest position.
- Thoroughly wash your hands.
- Document Resident Urinary Output on the Documentation Form.
- Recording in ml’s or cc’s.
- Ensure you are documenting in the OUTPUT section of the Documentation Form.
- Thoroughly wash your hands.
- Think about the Steps of your Skill. Make any corrections. Tell the evaluator your Skills is complete.
- Not reading and following the Care Plan, the whole Care Plan, and nothing but the Care Plan.
- Not greeting Resident or describing Task.
- Not following Barrier Rules throughout.
- Not recording Urinary Drainage.
- Not recording Urinary Drainage in the proper area on the form.
- Not recording Urinary Drainage in ml’s or cc’s.
- Not wearing gloves when handling Graduate Container.
- Allowing Urinary Drainage Port to touch Uniform.
- Allowing Urinary Drainage Port to touch the floor.
- Allowing Urinary Drainage Port to touch the Graduate Container.
- Not Returning Urinary Drainage Port to its holder.
- Not Completely emptying contents of Urinary Drainage Bag.
- Not covering Triangular Graduate Container while transporting.
- Not placing Triangular Graduate on a flat surface to Measure.
- Not Measuring Triangular Graduate on a flat surface or at eye level.
- Not properly cleaning Triangular Graduate.
- Not properly disposing of Graduate Container rinse water.
- Not Disposing of Barriers properly.
- Touching Resident with dirty hands.
- Not closing Privacy Curtain.
- Not ending Skill with clean hands.
