Measure and Record Respirations

Counting respirations, or breathing, is a commonly assigned vital sign. You cannot tell the patient that you are counting their respirations, and you cannot stare at their chest without holding their wrist. The patient needs to think you are taking their pulse, so they don’t alter their breathing patterns. You will calculate the respiration count over 1 minute – sometimes you will count for one whole minute and other times you can count for 15 seconds and multiply by 4. So, how do we know which method to use with which patient? Read the care plan!

You are graded on reading and following the care plan – it’s a big part of your grade! This care plan clearly instructs us to count the respirations for one whole minute, so you MUST count for one whole minute to pass. Since this care plan instructs us to measure AND RECORD the respirations, recording counts just as much as counting! If you don’t record the reading, you will fail. Since this care plan is very specific in these regards, following it will count big for the exam. Always read and follow the care plan!
